Read and submit the Participant Agreement below. Students may not participate without a signed agreement. For participants under age 18, this form must be completed by parent or guardian.
Participant Agreement
I have read and will follow Studio Policies and current COVID-19 Policies.
In regard to the services of Aerial Dance Chicago, their officers, employees, volunteers, participants and all other persons/organizations acting in any capacity on their behalf (which we will now collectively refer to as “ADC”), I hereby agree to release, indemnify, and discharge ADC, on behalf of myself and/or my children as follows:
I acknowledge that my participation in ADC activities involves various dance forms and entails unanticipated risks that could result in physical or emotional injury, including severe injury, paralysis and death. I understand that such risks simply cannot be eliminated without compromising the essential qualities of the activity. I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating, or else I agree to bear the costs of such injury or damage. My participation in this activity is purely voluntary, and I elect to participate despite the risks. I understand that I must judge my own capabilities in regard to these activities and agree to act within the limits of my own body. I take full responsibility for my judgments and agree to act with safety as the number one priority.
I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless ADC from any and all claims, demands or causes of action, which are in any way connected with my participation in this activity or my use of ADC’s equipment or facilities. Should ADC or anyone acting on their behalf, be required to incur attorney’s fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs. By signing this document, I acknowledge that if anyone is hurt during my participation in this activity, I may be found by a court of law to have waived my right to maintain a lawsuit against ADC on the basis of any claim from which I have released them herein.
Photo/Video Release: I understand that ADC intends to document activities through photo and video. By signing below I authorize ADC to photograph and videotape activities and use these images and videos for documentation and promotional purposes. I understand that I will not receive compensation of any type for these images/videos and I understand that their purpose is to generate support for ADC s unique activities by showing the community and funders the scope of ADC’S endeavors.
COVID-19 Disclaimer: An inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID-19 is a highly contagious disease that can cause severe illness and death. By attending programs at Aerial Dance Chicago, you voluntarily assume all risks related to exposure to COVID-19.
